Download and print the “Application Pack for Declaration of Nullity of a Marriage” (linked below) and fill in as much as possible, according to the instructions given in the pack.

Submit the “Fact Sheet” and “Preliminary Submission” along with the following documentation to the Tribunal for initial assessment:
 

          •      Baptism Certificate (if any)
          •      Previous Marriage Certificate
          •      Divorce Certificate
